The Bright Eyes MD Procedure in Action
On this patient, the Bright Eyes MD Procedure lifts and tightens the entire lower lid and upper cheek complex, eliminating the need for skin removal despite lower lid bags.
Brings eyes forward
Makes eyes look bigger
Lifts, fills upper lids
Brows lifted without touching
Excess fat removed upper, lower lids
Excess skin upper lid removed
No incisions lower lids
No skin removed lower lids
How is it possible no undereye skin needs to be removed ?
-
This patient had excess fat in both upper and lower lids, which was removed.
Excess skin in the upper lids was trimmed..
The lower lid skin being stretched out over the fat bulges would have caused loose skin folds once the fat was removed.
Conventional Eyelid Surgery would have used skin pinch incisions to trim the loose skin folds, leaving full width lower eyelid scars.
-
In Bright Eyes, smooth lower lid skin was achieved by lifting and tightening the lower lids and upper cheeks.
There was no loose lower lid skin left after tightening, so no skin removal was done, and the lids were left scar free.
Replacing foundational fat back underneath the eyes brought them up and forward, making them more prominent, and lifted the brows.
Patient is 77.
The Bright Eyes MD Procedure Does:
Bring the eyes forward
Lifts the eyes
Makes the eyes look larger
Fills out the upper lids
Lifts the brows
Removes under eye bags
The Bright Eyes MD Procedure Does Not:
How is this possible?
The Bright Eyes MD Procedure moves fat bulging into the lower lids, known as “under-eye bags,” back to its original, youthful position beneath the eyes, before it shifted into the lower lids.
Replacing the under-eye fat naturally lifts the eyes, upper lids, and brows up and forward, bringing deep-set eyes to the front to catch more light, making them look bigger, brighter, and more youthful—making this...

The Bright Eyes Procedure represents a paradigm shift in our understanding of the eye aging process, and its surgical approach.
Conventional Eyelid Surgery simply removes fat bulging into the lower eyelids.
Bright Eyes sees fat bulging into the lower eyelids as fat which was originally under the eyes, but was displaced forward due to loose connective tissue.
This fat is the underlying foundation for all of the orbital tissues.
-
As this foundational fat slips out of the orbit, the tissues above it, including the eyes, upper lids, and brows, fall down and back in the eye socket.
In the Before photo it is evident that the depressions in the upper lids are equal in volume to the bulges in the lower lids, consistent with the upper lid tissues falling down and back as the foundational fat bulges forward.
-
The Bright Eyes Procedure replaces foundational fat bulging into the lower lids back under the eyes where it was originally.
This lifts the eyes, upper lids, and brows up and forward into their youthful positions, reversing the eye aging process.
Also, it secures the foundational fat in place under the eyes, preventing the eye aging process from continuing to occur.
This patient, of normal body weight, had no excess fat or skin, so no removal was required.

Bright Eyes MD Fixes Conventional Eyelid Surgery Complications
This patient was referred to Eye Plastic Surgeon Dr. Mark Garbutt, creator of the Bright Eyes MD procedure, for surgery, by locally prominent Double Board Certified Facial Plastic Surgeon Maurice Sherman, after conventional eyelid surgery failed her.
Dr. Garbutt presented this patient, among others, to the Plastic Surgery Faculty at the UC San Diego Medical School, in a March 2022 Grand Rounds Lecture, and received the following compliments:
“On a scale of 1 to 10, your work is a 15.”
- Thomas Kennedy MD, Plastic and Reconstructive Surgeon
“Mark, you’re of course a very skilled surgeon…Your work is very important not only for the aesthetically concerned population, but also for the reconstructive patient population.”
- Mayer Tenenhaus MD, Professor of Plastic and Reconstructive Surgery, UCSD Medical School

Proof the Bright Eyes MD Procedure Moves the Eyes Up and Forward
After performing the Bright Eyes MD Procedure on the right eye, the surgery was paused to photograph the difference between the left, unoperated eye, and the right, operated eye.
The right eye is significantly more forward, and the right brow more elevated, than the left.
Simply securing the fat underneath the right eye caused it to move forward and the brow to raise.

Brooke Shields is a victim of the eye aging process and states she has not had plastic surgery.
-
Aging sets off a cascade of event which affects the eyes:
Connective tissue that supports the eyes and tightens the lids becomes loose;
Eyes, being heavy, press down on the fat underneath, making it bulge into the loose lower lids;
The eyes sink down and back behind the bulging fat, making them look smaller;
Upper lids and brows sink down and back with the eyes, collapsing into the eye sockets.
-
Conventional Eyelid Surgery only removes the bulging lower eyelid fat.
It does not reverse or prevent the cascade of eye aging, but makes matters worse by removing necessary structural fat, speeding up the aging process.
-
The Bright Eyes Procedure, by securing bulging lower lid fat bck under the eyes, reverses the entire cascade of eye aging, and prevents it from progressing.
Brooke Shields states that she has not had plastic surgery for fear it will "make her look like Spock."
The eye aging process is fully evident in her 2024 photo, giving a macabre effect.
Bret Baier

Conventional Eyelid Surgery cannot bring deep-set eyes forward.
It can only offer a brow lift, which combined with small, deep-set eyes, gives an unnatural look.
-
In 2014 Bret Baier had forehead wrinkles from using his forehead muscle to raise his brows.
Even so, he can barely raise his brows to a normal height, and still has outer brow droop.
It appears that his eyes have begun moving back in the eye sockets, which would cause the upper lids and brows to collapse downwards and inwards.
This would have required him to constantly use his forehead muscle to raise his brows, with consequent forehead wrinkles even at rest.
-
In 2024, his eyes are even further back, making them look small and beady.
If he had conventional eyelid surgery, it was disastrous.
His upper lids are not visible, and his inner brows are entering the orbits.
Botox in the central forehead may also play a role.
His outer brows, however, are well above the orbital rim, in contrast to his 2014 photo, indicating a temporal brow lift, giving a "Spock" - like or "seagull" look.
Most conventional brow lifts, performed with conventional eyelid surgery, just lift the outer brows, and let the inner brows fall, giving this kind of appearance.
If the brows are fully lifted, it will still give a "surprised" look as the eyes are still so small that a normal brow height seems abnormally high.
The Bright Eyes Procedure would have given a completely natural brow lift, without need for a separate procedure, restored his upper lids to their natural fulness, and brought the eyes forward to their youthful prominence, reversing his small, beady eye look.
The Bright Eyes MD Procedure Sets a New Standard in Cosmetic Eyelid Surgery
Is the first and only cosmetic eyelid procedure that:
Brings the eyes up and forward
Restores upper lid fulness without fat or filler injection
Lifts both the inner and outer brows without a separate brow lift sugery
Lifts and tightens lower lids and upper cheeks, resolving skin folds and excess fluid
No lower lid incisions or scars
